211 LA was a critical connector during the COVID 19 pandemic, linking individuals and families to food, Medi Cal, mental health care, and emergency housing support. It plays a vital role in ensuring that no matter the situation—aging, disability, unemployment, or crisis—people can get the right help at the right time.

211 LA partners with the LA County Board of Supervisors, LAHSA, DMH, DHS, AARP, SoCal Gas, and many more to bring coordinated care to residents across all eight Service Planning Areas (SPAs) in the county.

About the Organization
St. John’s Community Health is a trusted network of community health centers serving Los Angeles, San Bernardino, and Riverside Counties. Since 1964, this nonprofit, federally qualified health center (FQHC) has been committed to delivering affordable, accessible, and high-quality care for individuals and families—especially those who are low-income, uninsured, underinsured, or facing economic or social hardships.

From a one-room pediatric volunteer clinic, St. John’s has grown into one of the largest FQHC providers in the region, with over 20 service locations, including mobile units, school-based wellness centers, clinics in public housing, and standalone medical facilities.
